    Job Description
    Pathways Center offers Peer and Psychosocial Rehabilitation (PSR) services based out of outpatient clinics around the company.  Psychosocial Rehabilitation services include the development of environmental and recovery supports considered essential in improving a person's functioning, learning skills to promote the person's self-access to necessary services, and creating environments that promote recovery and support the emotional and function improvement of the individuals served. 
     
    Pathways Center also provides Community Access Group (CAG) services to individuals diagnosed with intellectual and/or developmental disabilities (IDD).  The Community Access Group services are designed to provide oversight, assist with daily living, socialization, communication, and mobility skills building and supports in a group. These services may be provided in a facility or a community as appropriate for the skill being taught or specific activity supported.  The purpose is to provide assistance in acquiring, retaining, or improving socialization, and adaptive skills for active community participation and independent functioning outside the individual’s own or family home, such as assisting the individual with money management, teaching appropriate shopping skills, using public transportation, and teaching nutrition and diet information
     
    Pathways Center’s Care Campus has two 24/7 Crisis Stabilization Units; one for adults and one for individuals ages 5-17.  Pathways Center's Crisis Stabilization Units (CSU's) are short term (2-7 days), medically monitored crisis residential programs, which offer services for a limited duration to stabilize acute psychiatric or behavioral symptoms, evaluate treatment needs, and develop plans to meet the needs of persons served.  The programs operate 24 hours per day 7 days per week. The CSU's are designed to serve as a first line of community based alternative care to hospitalization, offering psychiatric stabilization and detoxification services on a short-term basis. 
     
    • Under supervision of the Director of Client Engagement, the Client Curriculum Coordinator plans, writes and implements curriculum related to the goals of the Peer, PSR and IDD CAG programs. The Client Curriculum Coordinator may also assist with groups and activities at Pathways Center’s Care Campus Adult Center and Child Center.   Responsibilities include but are not limited to the following:
    •  
    • Considers individual goals, personal observation, and talks with the patient and his or her loved ones to help ensure the treatment and recovery experience is impactful, meaningful, and enjoyable
    • Works with Director of Client Engagement, Client Activities Associates and other healthcare team members to successfully facilitate skills-based activities and groups
    • Stays informed and current on communal and therapeutic resources available to best meet each patients needs
    • Documents each patient’s responses to the various therapeutic activities conducted by the Client Curriculum Coordinator
    • Visits each Peer/PSR/IDD location a minimum of once per month to gather feedback from Clients on what they would like to include in the curriculum
    • Partners with Director of Client Engagement and AnotherWay Foundation in various client and staff events
    • Based on individual goals and feedback from Clients, writes curriculum to help Clients meet goals and learn new skills
    • Acts immediately on any crisis, following protocol and basic CPR/first aid training
     
    The individual in this role must be organized and demonstrate the ability to communicate effectively to mobilize team members to execute planned activities and written curriculum.
     
    Qualifications - External
    Bachelor's degree in a related field from an accredited college or university AND One year of experience in the area of assignment OR Associate's degree from an accredited college or university OR Vocational/Technical degree in area of assignment from an accredited college AND Three years of experience in the area of assignment OR One year of experience required at the lower level Practical Instructor 3 (EDT012) or position equivalent.
